Distal versus traditional radial access in emergency angiography or percutaneous coronary intervention: a systematic review / Acesso distal versus radial tradicional na angiografia de emergência ou intervenção coronária percutânea: uma revisão sistemática

Objetivo: comparar a eficácia e segurança do acesso radial distal versus tradicional em intervenções coronárias de emergência. Metodologia: revisão sistemática (OSF: 10.17605/OSF.IO/R9YMG) nas bases PubMed, Embase e Cochrane, incluindo ensaios clínicos e estudos observacionais comparativos no contexto de urgência. Resultados: foram incluídos oito estudos. O acesso radial distal associou-se a menor incidência de oclusão da artéria radial e tempo reduzido de hemostasia. Embora o acesso radial tradicional tenha apresentado maior tempo de punção e taxas discretamente superiores de falha inicial, não houve diferenças clinicamente relevantes nos tempos porta-balão ou porta-fio em pacientes com infarto agudo do miocárdio. Conclusão: o acesso radial distal é uma alternativa segura e factível ao acesso radial tradicional em procedimentos de emergência. A técnica oferece vantagens superiores na preservação vascular sem comprometer a eficácia ou a agilidade da reperfusão miocárdica.
