Objective: To analyze the scientific evidence regarding the bioethical issues involved in family refusal for organ donation. Method: Integrative review conducted in March 2024, using the following information databases: BIREME, LILACS, PubMed/Medline, and SciELO. The search strategy was developed through the Health Sciences Descriptors (DeCS) and the terms available in the Medical Subject Headings (MeSH). Results: After a full analysis, 13 studies that explored the ethical, moral, and cultural reasons behind family refusal for organ donation were selected to compose the final sample of this research. Several factors contributing to family refusal were evidenced, including lack of knowledge of the potential donor’s wishes, religious issues, inadequate preparation of the healthcare team to handle the situation, and ethical and moral concerns related to the donation process. Conclusion: The bioethical approach to analyzing family refusal for organ donation highlights the complexity and the importance of considering ethical and moral values in clinical practice and health policies.
